Lichen planopilaris associated with oral lichen planus *

  A 63-year-old Japanese woman visited the Department of Dermatology of our hospital with cicatricial alopecia and erythematous macules on her head. She also presented with oral lichen planus. Histological examination of the erythematous macules revealed hypergranulosis, liquefaction degeneration of the epidermal basal cell layer, Civatte bodies and an infiltration of lymphocytes into the upper dermis. Follicular plugging and infiltration of lymphocytes in a hair follicle were also observed. Direct immunofluorescence examination disclosed IgM-positive Civatte bodies and a deposition of fibrinogen at the dermal-epidermal junction. A diagnosis of lichen planopilaris was made on the basis of clinical signs and histopathological findings. Treatment with clobetasol propionate solution improved her erythema and stopped loss of hair. (Accepted on August 10,2004)
