Osteochondral fracture of the talus with lateral collateral ligament injury of the ankle joint *

A case of osteochondral fracture of the talus with lateral collateral ligament injury of the ankle joint is reported. A 22-year-old male visited our division with a chief complaint of right ankle pain. He sustained a lateral collateral ligament injury of the right ankle, and he was treated with a cast for three weeks by another doctor. On his first visit, instability of the right ankle joint was noted with a talar-tilt-angle of 15 degrees. There was no evidence of any lesion of the talus radiographically. He was followed without further treatment and sustained an inversion sprain again, which resulted in a gait disturbance. Magnetic resonnance imaging(MRI) studies were performed. These revealed intensity change in the anterolateral aspect of the talar dome. A diagnosis of osteochondral fracture of the talar dome was made, and the patient was treated conservatively. It is necessary to consider complication of osteochondral fracture of the talus when a patient experience continued pain after a ligament injury of the ankle joint or when the talar-tilt-angle is over 15 degrees. An MRI study may be the examina-tlon of choice for the diagnosis. (Accepted on October 20, 1937)Kawasaki Igakkaishi 23( j ) : 191-195, 1997
